Measure your waist, enter it with your height in the same units, and read the ratio. The threshold is the same for everyone, which is most of the point.

How to measure the waist

The number is only as good as the measurement, and this is where most errors come from. Measure directly against the skin, at the level of the navel — roughly midway between the lowest rib and the top of the hip bone. Stand relaxed, breathe out normally, and do not pull the tape tight or hold your stomach in. The tape should be level all the way round.

Do it at the same time of day when comparing over weeks, ideally in the morning. A meal moves the measurement by a centimetre or two, which is enough to shift the ratio and mean nothing at all.

The boundaries

  • Under 0.4 — below the range usually studied. Worth mentioning to a doctor rather than treating as a good score.
  • 0.4 to 0.5 — the healthy range in most published work.
  • 0.5 to 0.6 — increased risk; this is where guidance suggests taking action.
  • 0.6 and above — substantially increased risk.

The reason one threshold works for everyone is that dividing by height already adjusts for body size. BMI needs separate cutoffs for South and East Asian populations, because the same BMI corresponds to a higher level of risk in those groups. Waist-to-height needs no such adjustment, which makes it easier to use and harder to misapply.

What this measure does not do

It is a screening indicator, not a diagnosis, and it has real limits worth stating plainly:

  • Not valid during pregnancy, and not appropriate for children against these adult boundaries — children need age-and-sex percentile charts.
  • It cannot see inside you. Waist circumference includes both the fat under the skin and the fat around the organs, and only the second carries most of the risk. It is a good proxy, not a measurement of the thing itself.
  • It says nothing about fitness, blood pressure, blood lipids or family history, all of which matter and none of which a tape measure reaches.

A ratio above the boundary is a reason to have a conversation with a doctor who can check the things this cannot, not a verdict on its own. A ratio below it is reassuring about one specific risk factor and silent about the rest.
